Citadel Healthcare seeks a licensed Practical Nurse (LPN) to care for residents at our Chicago facility. The role includes assessments, care planning, and coordinating with the IDT to ensure high-quality, compliant nursing care.

Candidates should have an active Illinois LPN license, a nursing degree or diploma, and ideally 1 year in long-term care; the job offers supportive teamwork, flexible scheduling, and growth opportunities.

Qualifications

• Nursing degree or diploma from an accredited college.
• Current license to practice in Illinois as an LPN.
• 1 year of experience preferred.
• Long-term care experience preferred.

Responsabilités

• Provides licensed care to residents as ordered by physician and per standards.
• Performs resident assessments/observations and reviews care plans.
• Monitors vital signs and administers IV therapy within license limits as ordered.
• Documents care in nurse's notes reflecting resident responses.
• Maintains safe environment and addresses safety risks promptly.
• Directs non-licensed staff to meet professional standards of care.
• Communicates with residents, families, physicians, and staff as required.
• Ensures care plans are implemented and updated per professional standards.
• Ensures infection control standards are followed and non-compliance addressed.
• Follows HIPAA policies and other company procedures.
• Performs other duties as assigned.
