Job summary

Citadel Healthcare in Freeport, IL seeks a Licensed Practical Nurse (LPN) to join our team. The LPN provides licensed care to residents, administers medications, monitors vital signs, and collaborates with physicians and the IDT to ensure high-quality, compassionate care. This role emphasizes safety, documentation, and adherence to state and federal standards in a long-term care environment.

Candidates should have active LPN license, CPR certification, and ideally one year of LTC experience.

Qualifications

• Current CPR Certification required.
• Possess a current license to practice in the State as a Licensed Practical Nurse.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provide licensed care to residents as ordered by physician and in accordance with standards.
• Provide and receive nursing reports at shift changes.
• Maintain clean and sanitary work areas and patient rooms.
• Direct and supervise CNAs to ensure care is rendered per standards.
• Perform resident assessments and review care plans.
• Take vital signs and administer medications and IV therapy within license.
• Transcribe physician orders to MAR and charts.
• Complete admission, transfer and discharge forms.
• Ensure safety and address potential hazards.
• Report on shift activities and contribute to care plan changes.
