Job summary

Avento Health seeks a Medical Case Manager / Licensed Practical Nurse (LPN) to support adults with intellectual and developmental disabilities in community-based residential settings around Peoria, IL. The role blends practical nursing with care coordination and documentation to ensure comprehensive health supports.

You will work with interdisciplinary teams, advocate for individuals, and help connect them to medical services while navigating community resources.

Qualifications

Current, unrestricted Illinois LPN license.
Strong clinical judgment with ability to elevate changes in health status.
Ability to coordinate care across providers and settings.
Strong written documentation, organization, and communication skills.
Collaborate effectively with individuals, families, and teams.

Responsibilities

Coordinate and monitor health-related services for adults in community-based settings.
Collaborate with nursing staff, physicians, guardians, and direct support professionals.
Assist with medical care planning and ongoing health-related supports.
Monitor health status and communicate concerns to clinical providers.
Coordinate medical appointments, referrals, and follow-up care.
Maintain accurate physician orders, medication information, and health records.
Travel locally between residences and service locations as needed.
