Nonotuck Resource Associates, Inc. is seeking a Licensed Practical Nurse (LPN) to lead case management, intake, and assessment processes for new referrals. You will develop care plans, coordinate with healthcare teams, and monitor member health through on-site visits and documentation.

Qualifications

• Current MA Licensed Practical Nurse (LPN) license required.
• Strong computer and organizational planning skills.
• Two years' recent experience with elders or individuals with ID/DD/ABI.
• Excellent interpersonal and communication abilities.
• Valid driver's license with a clean RMV record.

Responsibilities

• Intake and Assessments: complete intake assessments and subsequent assessments for new referrals.
• Supervision and Training: provide supervision and evaluation of caregivers.
• Individualized Plans of Care: develop and maintain detailed plans tailored to members' needs.
• On-Site Visits: conduct visits and monitor health status and needs.
• Reporting and Documentation: report changes, complete nursing notes, and document per service type.
• Discharges/Transitions: plan and implement transitions in coordination with the health team.
• Education and Training: provide ongoing health education to members and caregivers.
• On-Call System: participate in on-call coverage as scheduled.
