Job summary

  • Ability to work independently while collaborating with an interdisciplinary care team.

Responsibilities

  • Provide nursing care and health-support services in accordance with professional standards and applicable requirements.
  • Assess individuals experiencing illness, injury, or changes in physical or mental condition and provide appropriate nursing guidance.
  • Serve as a clinical resource to direct-care staff regarding medical concerns and individual health needs.
  • Train and oversee staff in medication administration and individualized health-support procedures.
  • Provide education related to medical conditions, specialized diets, choking risks, adaptive medical equipment, seizure disorders, and other health needs.
  • Coordinate medical, laboratory, dental, and other healthcare appointments and follow-up needs.
  • Administer scheduled and PRN medications when required and in accordance with written physician orders.
  • Review physician orders and medication records for accuracy and completeness.
  • Coordinate prescription and medication needs with pharmacies and healthcare providers.
  • Monitor changes in physical health, mental status, medication response, and overall condition.
  • Complete nursing assessments, health-risk documentation, nursing notes, and other required clinical records accurately and on time.
  • Communicate relevant changes in condition to healthcare providers and appropriate members of the care team.
  • Support applicable state, federal, and quality requirements.
  • Provide nursing guidance during urgent situations and participate in clinical problem-solving as needed.

Registered Nurse (RN) - Community-Based Care | Arlington Heights, IL

Avento Health is recruiting a Registered Nurse for a full-time, community-based nursing opportunity in the Arlington Heights area. This position is ideal for an RN who enjoys combining clinical nursing, care coordination, medication oversight, health advocacy, and staff education outside of a traditional bedside environment.

The RN will support adults with intellectual and developmental disabilities in community-based settings while serving as a clinical resource to direct-care staff. The position includes local travel throughout nearby communities and requires the ability to work independently while collaborating with healthcare providers, pharmacies, families, guardians, and other members of the care team.

Schedule

Monday through Friday, 9:00 AM to 5:00 PM, with significant flexibility based on appointments, program priorities, individual needs, and clinical responsibilities. Occasional on-call consultation and response to urgent situations may be required.

Compensation

$85,000 annually. Compensation may be higher based on experience and qualifications, including Illinois DHS Nurse-Trainer qualification.
