Medical Case Manager / Licensed Practical Nurse (LPN) — Peoria, IL

We are seeking a Medical Case Manager / Licensed Practical Nurse (LPN) to support adults with intellectual and developmental disabilities in community-based residential settings throughout the Peoria, Illinois area.

This position combines practical nursing experience with care coordination, health documentation, advocacy, provider communication, and collaboration with an interdisciplinary support team. It may be a strong fit for an LPN who enjoys relationship-based nursing and wants to work outside of a traditional bedside environment.

What You’ll Do
  • Coordinate and monitor health-related services for adults receiving community-based residential supports.
  • Collaborate with nursing staff, physicians, guardians, direct support professionals, and other interdisciplinary team members.
  • Assist with individualized medical care planning and ongoing health-related supports.
  • Monitor changes in health status and communicate concerns to the appropriate clinical provider.
  • Coordinate medical appointments, referrals, testing, follow-up care, and communication with outside providers.
  • Help maintain accurate physician orders, medication information, treatment instructions, and health records.
  • Review health documentation for completeness, accuracy, and appropriate follow-through.
  • Support medication oversight and health monitoring within the LPN scope of practice and applicable policies.
  • Provide guidance to direct-care staff regarding individual health needs and medical instructions.
  • Advocate for individuals and help connect them with appropriate medical services and community resources.
  • Participate in interdisciplinary meetings and contribute relevant medical information to individual service planning.
  • Maintain timely and accurate clinical and case-management documentation.
  • Travel locally between community residences, appointments, and other service locations as needed.
Anticipated Qualifications

  • Current, unrestricted Illinois Licensed Practical Nurse (LPN) license.
  • Strong clinical judgment and the ability to recognize and appropriately elevate changes in health status.
  • Ability to coordinate care across providers, appointments, and service settings.
  • Strong written documentation, organization, communication, and follow-up skills.
  • Ability to collaborate effectively with individuals, families, caregivers, and interdisciplinary teams.
  • Valid driver’s license and reliable transportation for community-based work.
  • Ability to work independently while maintaining appropriate communication with supervising clinical staff.
Preferred Background
  • Experience supporting adults with intellectual or developmental disabilities.
  • Experience in case management, care coordination, community nursing, home health, residential services, behavioral health, long-term care, rehabilitation, or chronic disease management.
  • Experience coordinating medical appointments, physician orders, treatment plans, or medication-related follow-up.
  • Experience educating or supporting direct-care staff.
Work Environment

This is a community-based position supporting individuals in residential and other service settings throughout the Peoria area. The role includes care coordination, documentation, communication with providers and caregivers, in-person support, and local travel.
