Opengate, Inc. is seeking an LPN to support comprehensive healthcare services for individuals served. The LPN will collaborate with RNs and the IDT, monitor health needs, administer meds, and assist with phlebotomy and routine bloodwork across residences and program sites.

Responsibilities include coordinating care, documenting changes, and ensuring regulatory compliance while providing compassionate, person-centered nursing support in residential and community settings.

Qualifications

• Current New York State Licensed Practical Nurse (LPN) license in good standing.
• Minimum one year of nursing experience preferred.
• Experience working with people with intellectual and developmental disabilities preferred.
• Demonstrated competency in medication administration, documentation, and healthcare coordination.

Responsabilités

• Administer medications and treatments as ordered by a physician and delegated by a Registered Nurse in accordance with professional standards and agency policy.
• Visit residences, day programs, and other service locations as assigned to monitor health status.
• Observe, document, and communicate changes in health conditions to the IDT.
• Review healthcare records and lab results for accuracy and follow-up.
