Job summary

Caregiver, Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities in residential and community-based settings. You will work under RN supervision to provide high-quality care and health support.

The role emphasizes person-centered care, collaboration with an interdisciplinary team, and adherence to infection control practices. Travel within the assigned area and direct client interaction are part of the job.

Qualifications

  • Active LPN license
  • Graduate of an accredited LPN program
  • Current BLS certification
  • Valid driver’s license and reliable transportation

Responsibilities

  • Administer medications and treatments
  • Monitor health conditions and report changes to clinical leadership
  • Assist individuals with activities of daily living (ADLs)
  • Maintain accurate clinical documentation and medication records
  • Support individualized healthcare and behavioral support plans
  • Promote a safe, respectful, and person-centered environment

  • Respond to medical emergencies as needed
  • Reinforce infection control and basic healthcare practices with support staff.

What You’ll Do

As an LPN, you will work under the supervision of a Registered Nurse to provide quality nursing care and health support for individuals receiving services in residential and community-based settings.

Required Qualifications
  • Active LPN license
