Job summary

Caregiver, Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities in residential and community-based settings. You will work under RN supervision to provide high-quality care and health support.

The role emphasizes person-centered care, collaboration with an interdisciplinary team, and adherence to infection control practices. Travel within the assigned area and direct client interaction are part of the job.

Qualifications

Active LPN license
Graduate of an accredited LPN program
Current BLS certification
Valid driver’s license and reliable transportation

Responsibilities

Administer medications and treatments
Monitor health conditions and report changes to clinical leadership
Assist individuals with activities of daily living (ADLs)
Maintain accurate clinical documentation and medication records
Support individualized healthcare and behavioral support plans
Promote a safe, respectful, and person-centered environment
Participate in interdisciplinary team collaboration
Respond to medical emergencies as needed
Reinforce infection control and basic healthcare practices with support staff.

Licensed Practical Nurse (LPN)

What You’ll Do

As an LPN, you will work under the supervision of a Registered Nurse to provide quality nursing care and health support for individuals receiving services in residential and community-based settings.

Responsibilities Include:

Administer medications and treatments
Monitor health conditions and report changes to clinical leadership
Assist individuals with activities of daily living (ADLs)
