Job summary

Caregiver seeks a compassionate LPN/LVN to join our team and provide hands-on care to individuals with IDD under RN guidance. You will support daily health needs, administer medications, and help ensure a safe, supportive environment.

Requirements include an active LPN/LVN license, graduation from an accredited program, 1+ year of nursing experience, and familiarity with EHR. BLS and a valid driver’s license are required.

Qualifications

• Active LPN/LVN license (multi-state preferred if applicable).
• Graduate of an accredited LPN/LVN program.
• 1+ year of nursing experience preferred.
• Experience with IDD, developmental disabilities, or long-term care preferred.
• Strong attention to detail and observation skills.
• Ability to work independently and communicate effectively.
• Familiarity with EHR systems and basic computer tools.
• Valid driver's license and reliable transportation.
• BLS certification.

Responsibilities

• Provide routine nursing care and health monitoring.
• Administer medications and treatments as directed.
• Observe and report changes in health or behavior.
• Assist with daily living activities (ADLs) like hygiene, mobility, and nutrition.
• Support implementation of individualized care and behavior plans.
• Maintain accurate documentation and medication records.
• Ensure proper medication storage, handling, and inventory.
• Respond to emergencies and provide immediate care when needed.
• Collaborate with RNs, providers, and interdisciplinary teams.
