Job summary

Opengate is seeking a Licensed Practical Nurse (LPN) to join the Interdisciplinary Team in Somers, NY. The LPN will administer medications, monitor health needs, coordinate services, and support residents across residential, day, home, and community settings.

This role requires NY LPN licensure, strong communication, ability to work independently and with a team, and willingness to travel between agency locations.

Qualifications

• Current New York State Licensed Practical Nurse (LPN) license in good standing.
• Minimum one year of nursing experience.
• Experience working with people with intellectual and developmental disabilities preferred.
• Ability to travel between agency locations and use electronic health records.

Responsibilities

• Administer medications and treatments as ordered by a physician and delegated by a Registered Nurse.
• Monitor health status across residences, day programs, and other service locations.
• Document, communicate changes in health conditions and care needs to the IDT.
• Coordinate healthcare services and participate in interdisciplinary team meetings.
• Assist with phlebotomy, blood draws, and laboratory specimen collection as required.
• Maintain accurate healthcare documentation and records in accordance with policies and regulatory requirements.
• Support continuity of healthcare operations during staffing shortages and emergencies.
