Qualifications

• Current NYS LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Experience with intellectual and developmental disabilities preferred.
• Competent in medication administration and documentation.
• Valid NY driver's license and ability to travel.
• Strong communication and teamwork skills.
• Proficiency with electronic health records.

Responsibilities

• Administer medications and treatments as ordered by a physician.
• Monitor health status during visits to residences and programs.
• Document changes and communicate with the IDT.
• Coordinate healthcare services and assist with scheduling.
• Complete routine bloodwork and phlebotomy for assigned individuals.
• Maintain accurate healthcare records per policy and regulation.
• Collaborate with RNs and participate in team meetings.
