Job summary

Opengate, Inc. in Somers, NY, is seeking a Full Time Licensed Practical Nurse (LPN) to support comprehensive healthcare services for people with intellectual and developmental disabilities across residential, day programs, and community settings.

The LPN will administer medications, monitor health, coordinate care with the Interdisciplinary Team (IDT), perform routine bloodwork and phlebotomy, and maintain documentation to support ongoing wellness and treatment plans.

Qualifications

Current New York State LPN license in good standing.
Minimum one year of nursing experience preferred.
Experience with intellectual and developmental disabilities preferred.
Competency in medication administration, documentation, and healthcare coordination.
Phlebotomy competency or ability to complete agency training.
Valid NYS driver's license and ability to travel between locations.

Responsibilities

Administer medications and treatments as ordered by a physician and delegated by a RN.
Monitor health status during visits to residences and program sites.
Communicate changes in health conditions to the IDT and team.
Review medical records and follow up as needed.
Collaborate with RNs on care plans and interventions.
Coordinate scheduling and attendance at medical appointments.
Document healthcare activities per policy and regulatory requirements.
Perform phlebotomy and routine blood draws.
Ensure requisitions and specimens are accurately completed.
