Haven Home Health & Hospice is seeking a compassionate Clinical Coordinator (LPN) to join our team in Norman, OK. This role links patients, caregivers, physicians, and clinical staff to ensure quality care and seamless communication.

The position requires an Oklahoma LPN license and prior home health or clinical coordination experience is preferred. Excellent communication and organization are essential, with multitasking in a fast-paced environment and reliable transportation.

Responsibilities

Coordinate patient care and communication between physicians, clinicians, patients, and caregivers.
Process physician orders and patient care updates.
Assist with patient admissions and ongoing care coordination.
Monitor and communicate changes in patient condition.
Maintain accurate patient records and documentation.
Support clinicians with scheduling, orders, and care planning.
Participate in interdisciplinary team communication.
