Job summary

Northern Arizona Hospice, part of the UnitedHealth Group family, is hiring an LPN as Clinical Care Coordinator to manage home-based patient care coordination. You will assist with order tracking, referrals, and scheduling to ensure seamless care and regulatory compliance.

The role emphasizes collaboration with RN Case Managers and Social Workers, with a focus on timely, compassionate support for patients and families in various care stages. A comprehensive benefits package is provided.

Qualifications

• Current LPN/LVN license in state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Helps manage order tracking, IDG preparation, and tasks related to patient care management.
• Monitors referrals daily and assigns RN/APN for start of care visits.
• Monitors productivity and distribution of visits; communicates changes to clinical leader as appropriate.
• Schedules nursing visits to RN Case Manager and Social Worker to support continuity of care.
• Participates in patient care team meetings to facilitate timely care during crises or transitions to end of life.
• Coordinates and tracks visits to ensure hospice policy compliance and regulatory requirements.
