Responsibilities

• Coordinate patient care and communication between physicians, clinicians, patients, and caregivers.
• Process physician orders and patient care updates.
• Assist with patient admissions and ongoing care coordination.
• Monitor and communicate changes in patient condition.
• Maintain accurate patient records and documentation.
• Support clinicians with scheduling, orders, and care planning.
• Participate in interdisciplinary team communication.
