Job summary

Haven Home Health & Hospice in Norman, OK is seeking a compassionate and organized Clinical Coordinator (LPN) to join our team. The Clinical Coordinator serves as a vital link between patients, caregivers, physicians and clinical staff to ensure quality patient care and effective communication.

This on-site role coordinates patient care, processes physician orders and patient care updates, and assists with admissions and ongoing care coordination.

Qualifications

• Current Oklahoma LPN license required.
• Home Health experience preferred.
• Previous clinical coordination experience preferred.
• Strong communication and organizational skills.
• Ability to multitask in a fast-paced environment.
• Proficient computer and documentation skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate patient care and communication between physicians, clinicians, patients, and caregivers
• Process physician orders and patient care updates
• Assist with patient admissions and ongoing care coordination
• Monitor and communicate changes in patient condition
• Maintain accurate patient records and documentation
• Support clinicians with scheduling, orders, and care planning
• Participate in interdisciplinary team communication
• Assist with orientation and training of new staff
• Participate in on-call, weekend, and holiday rotations as assigned
