Job summary

Care Initiatives is seeking a Licensed Practical Nurse (LPN) for a Charge Nurse role with part-time days and every other weekend. Competitive hourly rate plus shift differential is offered across forty skilled nursing communities in Iowa.

The LPN will perform assessments, administer medications, supervise CNAs, and coordinate care with interdisciplinary teams, all while ensuring regulatory compliance and compassionate resident-centered care.

Qualifications

• Licensed Practical Nurse (LPN) license in Iowa in good standing.
• Current CPR certification.
• State-approved LPN Supervisory Course or ability to obtain within first 90 days of employment.
• Knowledge of federal and state long-term care regulations and laws.
• Strong commitment to compassionate, patient-centered care.

Responsibilities

• Conduct ongoing assessments of residents' clinical status and contribute to individualized care planning with the interdisciplinary team.
• Deliver and oversee direct nursing care, including wound treatments, IV therapy (as permitted), catheter care, and other skilled interventions.
• Administer medications and prescribed treatments safely and document accurately.
• Serve as a clinical leader on the unit, coordinating resident care and communicating with physicians, leadership, residents, and families.
• Supervise, mentor, and support CNAs and other staff to promote accountability and high-quality resident care.
• Ensure adherence to regulations, infection prevention, and Care Initiatives policies in a safe environment.
• Provide education and emotional support to residents and families about care plans and goals.
