Care Initiatives is seeking a compassionate Licensed Practical Nurse (LPN) to join as a Charge Nurse across our multiple Iowa communities. The role emphasizes delivering high-quality nursing care, supervising CNAs, and guiding care planning with the interdisciplinary team.

Qualifications

• LPN license valid in Iowa in good standing; CPR certification current.
• LPN Supervisory Course completed or obtainable within first 90 days of employment.
• Knowledge of federal and state long-term care regulations and laws.
• Strong commitment to empathy, respect, patience and discretion in resident care.

Responsabilités

• Conduct ongoing assessments of residents’ clinical status, including vital signs and condition changes, and contribute to individualized care planning with the interdisciplinary team.
• Deliver and oversee direct nursing care, including wound care, IV therapy (scope permitting), catheter care, and other skilled interventions per orders and protocols.
• Administer medications and treatments, monitor responses and adverse effects, and maintain timely documentation.
• Serve as a clinical leader on the unit, coordinating resident care and communicating with physicians, leadership, residents and families.
