Job summary

Heritage Oaks is seeking a Licensed Practical Nurse (LPN) in Georgia to deliver compassionate, patient‑centered care in our non‑profit, mission‑driven facilities. You will collaborate with RNs, CNAs, and the care team to meet care plans and ensure positive resident outcomes.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• Experience in long-term care preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.
