Job summary

Autumn Lane Health & Rehab in Georgia is seeking a dedicated Licensed Practical Nurse to join our caring team. You will work with RNs, CNAs and the interdisciplinary team to deliver high-quality, person-centered care.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• Strong clinical judgment and critical thinking skills.
• Excellent communication and teamwork abilities.
• Prior long-term care experience preferred, but not required.
• A passion for delivering compassionate, high‑quality care.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
