Job summary

CHSGa in Georgia is seeking a Licensed Practical Nurse (LPN) for a full-time role in our compassionate care team. You will work with RNs, CNAs and the interdisciplinary team to administer medications and provide person-centered care.

You will monitor residents, document accurately, and uphold high clinical standards in a supportive environment that values teamwork and respect.

Qualifications

• Active LPN license (Georgia state or eligible).
• Passion for delivering compassionate, high-quality care.
• Strong clinical judgment.
• Excellent communication and teamwork abilities.
• Reliability and a commitment to excellence.
• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
