Job summary

Four County Health & Rehab, a nonprofit facility, is seeking an LPN for the night shift to deliver high-quality care. You will collaborate with RNs, CNAs, and the interdisciplinary team in a compassionate, patient-centered environment.

The role emphasizes medication administration, monitoring, and documentation, with opportunities for growth and a supportive leadership team. Night shifts include shift differentials and weekly pay.

Qualifications

• Active LPN license in Georgia or eligibility to obtain one.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

What You Bring

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)

• Strong clinical judgment and critical thinking skills
• Excellent communication and teamwork abilities
• Reliability and a commitment to excellence
• Prior long-term care experience preferred, but not required
