Job summary

UnitedHealth Group seeks a detail-oriented LPN Central Intake Coordinator to support Home Health Central Intake Teams by triaging calls and entering referrals accurately. You will obtain essential patient demographics and clinical information, coordinate with Central Intake Nurses, and ensure smooth referral flows to branches.

In this role you will communicate effectively with patients, families, and referral sources, enter data into the mainframe, and participate in ongoing training to stay

Qualifications

• LPN licensure in the state of practice required.
• 1+ year experience in office procedures.
• 1+ year Insurance/Medical experience.

Responsibilities

• Answers phones and triages phone calls to Central Intake Teams.
• Effectively communicates patient/referral information to Central Intake Staff and branches.
• Obtains demographic and clinical information from clients and referral sources to compile referrals.
• Coordinates communication of referral information with Central Intake Nurses.
• Transfers High Tech/nurse specific referral calls to Central Intake Nurses.
• Triages incoming calls and messages from referral sources and relays messages or transfers to voicemail.
• Discretion and diplomacy when obtaining information from callers for persons not immediately available.
• Provides general information about agency services to patient families and referral sources as needed.
• Data enter Central Intake and branch referrals into the mainframe as needed.
• Attends workshops, training programs, and seminars to stay current with departmental changes.
• Manages patient ping feed throughout the day in applicable markets and disseminates data to appropriate staff.
