Well Care Health seeks an Intake Coordinator - LPN to respond to inquiries and initiate referral intake for patients. You will coordinate information flow between staff nurses, patients and physicians, and support problem solving across the team.

The role requires an LPN with an active North Carolina license, at least two years of nursing experience (home health preferred). Strong communication and computer literacy are essential for success in a fast-paced office setting.

Formación

• Two years nursing experience preferred, home health experience a plus.
• Must have valid NC nursing license and be eligible to practice.
• Good communication and organization are essential.

Responsabilidades

• Assess referrals as they arrive by phone or in the office.
• Collaborate with coordinators on referral disposition.
• Interpret reimbursement and coverage guidelines for referrals and the public.
• Communicate information per department policies and procedures.
• Maintain confidentiality and promote customer loyalty.
