UnitedHealth Group seeks a detail-oriented LPN Central Intake Coordinator to support Home Health Central Intake Teams by triaging calls and entering referrals accurately. You will obtain essential patient demographics and clinical information, coordinate with Central Intake Nurses, and ensure smooth referral flows to branches.

In this role you will communicate effectively with patients, families, and referral sources, enter data into the mainframe, and participate in ongoing training to stay

Qualifications

• LPN licensure in the state of practice required.
• 1+ year experience in office procedures.
• 1+ year Insurance/Medical experience.

Responsabilités

• Answers phones and triages phone calls to Central Intake Teams.
• Effectively communicates patient/referral information to Central Intake Staff and branches.
• Obtains demographic and clinical information from clients and referral sources to compile referrals.
• Coordinates communication of referral information with Central Intake Nurses.
• Transfers High Tech/nurse specific referral calls to Central Intake Nurses.
• Triages incoming calls and messages from referral sources and relays messages or transfers to voicemail.
• Discretion and diplomacy when obtaining information from callers for persons not immediately available.
• Provides general information about agency services to patient families and referral sources as needed.
• Data enter Central Intake and branch referrals into the mainframe as needed.
• Attends workshops, training programs, and seminars to stay current with departmental changes.
• Manages patient ping feed throughout the day in applicable markets and disseminates data to appropriate staff.

The LPN Central Intake Coordinator in Home Health provides administrative support to Central Intake Teams by adequately triaging incoming calls and entering referrals in operating system. Receives and communicates accurate referral information within Central Intake as well as to Branches.

Primary Responsibilities:

• Answers phones and appropriately triages phone calls to Central Intake Teams
• Effectively communicates patient/referral information to appropriate Central Intake Staff and branches
• Obtains demographic and clinical information from clients, physicians office and other referral sources to compile referrals
• Coordinates communication of referral information with Central Intake Nurses
• Transfers High Tech/nurse specific referral calls to Central Intake Nurses
• Triages incoming calls and messages from referral sources, etc. Relays messages as appropriate or transfer to voice mailbox
• Coordinator must exercise discrimination and diplomacy on obtaining information from callers for persons not immediately available
• Provides general information about agency services to patient family, and other referral sources as needed
• Data enter Central Intake and branch referrals into the mainframe as needed
• Attends and participates in workshops, continuing educational programs, seminars, etc. to keep abreast of current departmental changes
• Attends and participates in annual in-service training programs.Manages patient ping feed throughout the day, in applicable markets
• Disseminate patient ping data to appropriate PCM, field clinicians, or sales representative, in applicable markets
• All other duties as assigned

Required Qualifications:

• LPN licensure in the state of practice
• 1+ years experience in office procedures
• 1+ years Insurance/Medical experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.45 to $26.20 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
