Job summary

Qualifications

• Current State License as an LPN.

Responsibilities

• Screens referrals and coordinates admission process.
• Coordinates services with providers, billing reconciliation.
• Outreach to community referral sources.
• Participates in weekly team meetings with Medical Director.
• Completes home visits and quarterly PCP visits.
• Tracks hospital/ER visits and patient care pathways.
• Arranges transportation and documents care plans.
