Pruitthealth Corporation is seeking a Licensed Care Manager for Home First in Newton County. This role screens referrals, coordinates admissions, and collaborates with providers to ensure smooth service delivery.

The position requires an LPN license, CPR certification, and at least five years of nursing experience, with hospice or SOURCE care preferred. A valid driver’s license and auto insurance are required, and the incumbent may travel within the service area.

Qualifications

• Current state license as a Licensed Practical Nurse (LPN).
• Valid driver's license and automobile insurance liability.
• Graduate of a state-approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Minimum of five years of nursing experience; hospice or SOURCE care preferred.

Responsibilities

• Screen referrals and coordinate admission process.
• Coordinate services with providers and reconcile billing.
• Participate in team meetings and present new admits and discharges.
• Complete quarterly home visits and PCP visits with required documentation.
• Track hospital/ER visits and collect records for audits.
• Travel using personal vehicle; arrange transportation as needed.
• Train new hires and participate in on-call rotations.
• Communicate with regulatory and APS/CPS referrals.
