Job summary

PruittHealth is seeking a Licensed Practical Nurse with strong admission coordination and home health management skills to support the Home First program in Georgia. The role involves screening referrals, coordinating services with providers, and overseeing paperwork and assessments in member homes.

Candidates should have an LPN license in Georgia, a valid driver's license, and at least five years of nursing experience, with hospice or SOURCE care preferred. Travel to member homes is required.

Qualifications

• Current State LPN license in Georgia.
• Valid driver's license and automobile insurance.
• 5+ years nursing experience; hospice or SOURCE care preferred.
• Ability to coordinate admission processes and care plans.

Responsibilities

• Screens appropriate referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Outreach to community referral sources.
• Completes new admission paperwork and in-home assessments.
• Participates in weekly team meetings with Medical Director and reviews all new admits and discharges.
• Performs quarterly home and PCP visits; tracks hospital/ER visits and prepares reports.
• Coordinates with outpatient therapy centers and home health agencies; arranges transportation as needed.
• Trains new hires and participates in on-call rotation per CMS requirements.
