Qualifications

• Current State Licence as a Licensed Practical Nurse (LPN).
• Must have a valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred.

Responsibilities

• Screens referrals and coordinates admission process with providers and billing.
• Coordinates services with home health agencies, rehab centers, and outpatient therapy.
• Maintains records of hospital/ER visits and follows up on variances and discharge plans.
• Performs in-home reassessments, mileage/time sheets, and transportation arrangements.
• Trains new hires and participates in on-call rotation as required by CMS referrals.
