The role requires an LPN license, CPR, five years of nursing experience, and a valid driver's license. You will collaborate with providers, complete paperwork, conduct visits, and participate in weekly team meetings to ensure high-quality patient care.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred.

Responsibilities

• Screen referrals and coordinate admission process.
• Coordinate services with providers and billing reconciliation.
• Attend weekly team meetings with Medical Director and present new admits and discharges.
• Complete quarterly home visits and PCP visits with required documentation.
• Track hospital/ER visits and coordinate records with hospitals.
• Coordinate with outpatient therapy centers, home health agencies, and rehab teams.
