Job summary

Qualifications

Responsibilities

• Screen referrals and coordinate admission process.
• Coordinate services with providers and handle billing reconciliation.
• Engage with community referral sources and attend team meetings.
• Complete admissions paperwork, home assessments, and quarterly visits.
• Track variances, plan actions, and follow up to resolution.
• Travel using own vehicle and arrange transportation as needed.
• Participate in trainings and be available for on-call rotation.
