Job summary

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Must have a valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Minimum of five (5) years of nursing experience; hospice or SOURCE care preferred.

Responsibilities

• Screens referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Outreach to community referral sources.
• Participates in weekly team meetings and presents new admits and discharges.
• Completes quarterly home visits and PCP visits with required documentation.
• Tracks hospital/ER visits and coordinates with hospitals for records.
• Coordinates with outpatient therapy centers, home health agencies, and rehab services as needed.
• Performs annual reassessments in members' homes and reviews medications for appropriateness.
