Job summary

Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) in Massachusetts to collaborate with RNs and provide compassionate, quality nursing care focused on patient dignity and autonomy.

The LPN performs assessments, plans, implements, and evaluates care within a primary nursing model, documents in EMR, and supports culture change initiatives while mentoring other team members. Strong communication and teamwork are essential.

Qualifications

• Graduate of approved school of nursing.
• Current Massachusetts license as an LPN in good standing.
• Previous experience preferred.
• Understand the philosophy and support the model of care at HSL.
• Professional, proactive, collaborative and results-oriented.
• Strong organizational and analytical skills.
• Ability to innovate, think strategically and manage multiple projects.
• Motivated to learn and adaptable to change.
• Computer literacy required.
• Excellent English language skills, verbal and written.

Responsibilities

• Ensure care within safe professional nursing practice.
• Collaborate with MD/NP/social worker/therapists to meet patient needs.
• Adhere to evidence-based interventions for optimal outcomes.
• Practice in a primary nursing model with RN oversight.
• Support autonomy and individual choice in care planning.
• Keep care plan accurate and updated with RN input.
• Document care in the EMR.
• Initiate necessary reports (DPH, incidents).
• Identify resources to ensure safe, effective care.
• Delegate and supervise nursing assistants.
• Participate in governance and quality improvement activities.
• May assume charge nurse responsibilities with RN oversight.
