Job summary

Hebrew SeniorLife in Boston is looking for a Licensed Practical Nurse (LPN) to collaborate with a Registered Nurse in delivering high-quality nursing care and maximizing patient quality of life.

The LPN will follow the nursing process, participate in care planning, and serve as a role model and mentor to the clinical support team. A strong commitment to culture change initiatives and patient dignity is expected.

Qualifications

• Graduate of an approved school of nursing.
• Current Massachusetts license as an LPN in good standing.
• Previous experience preferred.
• Understand the philosophy and model of care at HSL.

Responsibilities

• Ensures care delivered is within the scope and established standards of safe professional nursing practice.
• Collaborates with multidisciplinary team to meet clinical needs and monitor outcomes.
• Maintains accountability for assigned patients with RN oversight of assessment and care planning.
• Documents care in EMR and initiates necessary reports.
• Participates in unit activities and serves as a coach/mentor to PCAs.
• If acting as charge nurse, provides supervisory oversight and creates assignments.
