Job summary

Hebrew SeniorLife in Boston is seeking a Licensed Practical Nurse who will collaborate with the RN to deliver high-quality, patient-centered care. The LPN will participate in care planning, documentation, and mentoring of staff in a dynamic clinical unit.

The role requires Massachusetts LPN licensure, strong communication, and teamwork, with on-site responsibilities and adherence to HIPAA and safety standards. Experience with EMR systems and a primary nursing model is preferred.

Qualifications

• Graduate of an approved school of nursing is required.
• Current Massachusetts LPN license in good standing.
• Understanding the model of care at Hebrew SeniorLife.
• Strong communication and teamwork skills.
• Ability to adapt to changing priorities and maintain sound judgment.

Responsibilities

• Provide nursing care within scope and standards with RN oversight.
• Collaborate with multidisciplinary team to meet patient needs and monitor outcomes.
• Document care in EMR; prepare reports as required (DPH, incident, concerns).
• Delegate and supervise nursing assistants; may serve as charge nurse with RN supervision.
• Participate in committees, governance, and quality-improvement activities.
• Support patient independence and dignity; mentor junior staff.
