Responsibilities include collaborating with MD/NP, other disciplines, documenting in EMR, ensuring safety, participating in governance, and supporting patient dignity and autonomy.

Qualifications

• Graduate of an approved school of nursing.
• Current Massachusetts LPN license in good standing.
• Previous experience preferred.
• Understand and support the model of care at Hebrew SeniorLife.
• Professional, proactive, collaborative and results-oriented.
• Excellent oral and written communication with good judgment.
• Solid organizational and analytical skills.
• Adaptable to changing priorities and able to manage multiple projects.

Responsibilities

• Provide nursing care within the scope and established standards.
• Collaborate with MD/NP, nurses, social worker, therapists and dietitian to meet patient needs.
• Implement evidence-based interventions for optimum outcomes.
• Maintain primary nursing model with RN oversight and plan of care.
• Document care in EMR and initiate necessary reports.
• Delegate and supervise care delivered by CNAs; mentor staff.
• Participate in governance and quality improvement initiatives.
• Assist with unit activities and support safety and HIPAA compliance.
• Attend rounds and assist with care planning and scheduling when acting as charge nurse.
