Monarch Healthcare Management is seeking an LPN Care Coordinator to support the nursing leadership, assist with care planning, and manage data collection for a designated unit. The role includes participating in assessments, updating care plans, and coordinating with the interdisciplinary team to ensure resident needs are met.

You will monitor conditions, report changes, and assist with staffing, documentation, and compliant care delivery under RN supervision.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Excellent communication, both verbal and written, with a focus on patient care.
• Preferred experience in healthcare settings like skilled nursing or assisted living.

Responsibilities

• Assist with care planning and data collection for residents.
• Contribute to assessments such as MDS and RAPS and update care plans.
• Collaborate with the interdisciplinary team to meet resident needs.
• Monitor resident conditions and report changes to RNs.
• Participate in care conferences and ensure physician orders are reflected in plans.
• Provide nursing care under RN supervision and support LPNs and CNAs.
