Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator in Minnesota to support nursing leadership and coordinate care on a designated unit. You will assist with care planning, assessments, and updating care plans while collaborating with RNs and the interdisciplinary team.

You will monitor resident conditions, report changes, help with staffing, and ensure accurate documentation, while maintaining licensure and pursuing ongoing education in a positive workplace culture.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Excellent communication (verbal and written) and ability to perform essential functions.
• Experience in healthcare settings is preferred; familiarity with Medicare coverage is a plus.
• Strong multitasking, adaptability, emotional intelligence, and teamwork.

Responsibilities

• Assist with care planning, data collection, and implementation of nursing processes.
• Support RNs, manage staffing needs, and ensure safe, competent care delivery.
• Maintain documentation and adhere to nursing policies and regulatory requirements.
