Job summary

Monarch Healthcare Management in Minnesota is seeking an LPN Care Coordinator to support the director and nursing leadership in managing a nursing unit, assisting with care planning, data collection, and implementing the nursing process. This role helps monitor resident data, contributes to assessments, and collaborates with the interdisciplinary team to ensure needs are met.

You will monitor resident conditions, report changes to RNs, participate in care conferences, and help ensure care plans

Qualifications

• Must hold a current Minnesota LPN license and CPR certification.
• Effective verbal and written communication; able to perform essential job functions.
• Experience in healthcare settings preferred (skilled nursing/assisted living) and knowledge of Medicare.

Responsibilities

• Assist with care planning, data collection, and nursing process implementation.
• Monitor resident conditions and report changes to RNs; participate in care conferences.
• Provide nursing care under RN supervision and support LPNs and aides; ensure accurate documentation.
• Assist with staffing needs and ensure safe, competent care delivery.
• Promote quality improvement and maintain licensure and ongoing education.
