Job summary

Hamilton Health Center is seeking an LPN Care Coordinator to support patient-centered care at our Newport Family Care location in Millersburg, PA. The role involves coordinating care for higher-risk patients, educating families, and collaborating with a multidisciplinary team to improve outcomes and reduce ER visits.

The position requires an LPN license in PA, 2+ years in community health or care coordination, and strong communication skills.

Qualifications

• High school diploma or GED.
• Graduate of an accredited LPN program.
• Current license to practice as an LPN in Pennsylvania.
• At least two years of community/public health or care coordination experience.
• Proficiency in basic data entry and computer skills.
• Strong interpersonal and communication skills.
• Ability to work collaboratively within an interdisciplinary care team.

Responsibilities

• Provide care coordination and case management for patients.
• Co-manage higher-risk patients through assessment and care coordination activities.
• Maintain ongoing interaction with patients and families to support adherence to treatment plans.
• Educate patients, families, and caregivers regarding the Patient-Centered Care Model and applicable care needs.
• Serve as a liaison regarding medical and dental benefits.
• Coordinate with providers and care team to support continuity and quality of care.
• Monitor progress, document patient status, identify barriers to care, and report updates.
• May conduct home and hospital visits as part of coordination responsibilities.
• Ensure HIPAA compliance and confidentiality of protected information.
• Collect data to support service improvement and monthly reporting.
