Qualifications

• Active unrestricted LPN license in Pennsylvania.
• Two years of nursing experience required.
• Behavioral health, care coordination or case management experience preferred.
• Experience with electronic health records required.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinates behavioral health services and patient care activities, including referrals, new patient appointments, follow-ups, telepsychiatry, and team communication.
• Assists patients in navigating behavioral health services with education, support, and resource coordination.
• Gathers clinical information, assists with medication refills within scope of practice, and communicates concerns to providers.
• Collaborates with providers, staff, and community resources to support coordinated, patient-centered care.
• Prepares patients for scheduled appointments and maintains documentation in the EHR.
• Participates in quality improvement activities and ensures confidentiality and compliance.
