The Wright Center for Community Health is seeking an LPN Care Coordinator for our outpatient care team. The role focuses on care coordination via telephone, supporting high-quality patient care and HIPAA compliance in a fast-paced environment.

You will manage 30+ high-risk patients, coordinate with PCP teams, handle follow-up visits, and contribute to ongoing quality improvement while working an evenings/weekends/holidays rotation. Licensed Practical Nurse with BLS required.

Qualifications

• Graduate of an accredited LPN program.
• Active Pennsylvania Practical Nursing license.
• BLS Certification required.
• Experience with electronic health records; Medent a plus.
• Excellent communication and teamwork skills.
• Commitment to process improvement and quality.
• EOS model familiarity preferred.
• Ability to work as part of a care team.
• Evenings/weekends/holidays rotation may be required.

Responsabilités

• Oversee care for patients transitioning from facilities with follow-up calls.
• Coordinate care for at least 30 high‑risk and rising‑risk patients.
• Arrange follow-up visits and manage patient data on symptoms and meds.
• Handle tasks including drawing meds for clinicians and monitoring devices.
• Answer calls, create orders, relay test results and schedule tests.
