Job summary

Carefree Living in St. Cloud, MN seeks a Licensed Practical Nurse (LPN) Care Coordinator to join our assisted living team. If you value resident-centered care, teamwork, and a supportive environment, we want to hear from you.

You will work Monday–Friday daytime, partnering with RNs, assist with ADLs, manage medications, contribute to care plans, document meticulously, and mentor staff. A Minnesota LPN license and strong communication skills are essential.

Qualifications

• Minnesota LPN license, active and in good standing.
• Excellent communication with residents, families and staff.
• Experience in Assisted Living or geriatric care preferred.

Responsibilities

• Assist residents with activities of daily living and scheduled services.
• Work with RNs to develop and review care plans and attend care conferences.
• Assist with admission processes and medication administration as directed.
• Document care accurately and maintain confidentiality.
• Mentor and supervise Resident Assistants and coordinate medication processes.
• Perform supervisory visits, chart reviews, and ensure policy compliance.
• Communicate clearly with residents, families, staff, and other providers.

Schedule & Compensation

Hours: Monday–Friday, daytime schedule Pay: Based on experience

Maintain excellent work/life balance while delivering high-quality care in a supportive workplace that values your contributions.

What You'll Do:

• Assist each client to reach and maintain his or her highest level of performance and independence by performing assistance with activities of daily living and implementing scheduled services as needed
• Participates with registered nurses in the development and review of care plans; participates in care conferences
• Assist with the admission process as directed
• Perform safe administration of medications and implementation of treatments and other therapies as directed
• Completing documentation with accuracy and attention to detail
• Supporting Resident Assistants through mentorship, coaching, and positive leadership Managing medication orders, refills, and overall medication processes
• Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License
• Demonstrate effective written and verbal communication with residents, family members, staff, and other health care providers. Maintain service excellence and a high level of confidentiality.
• Actively participate in agency-wide activities and its continuous quality improvement
• Understand the importance of seeking assistance as needed and demonstrate dependability and capability of following instructions. Adhere to policies, and procedures and participate in the organization's quality improvement program.

What We're Looking For

• Current Minnesota LPN license
• Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
• Experience in Assisted Living highly preferred
• Familiarity with Alzheimer's/dementia care is a plus
• Confidence using electronic documentation systems
• A team-oriented mindset and desire to contribute to a supportive work culture
