LPNResponsibilities:
- Assures residentsmaintainadmission criteria,monitor, and recommend alternative placement if necessary.
- Assures residents have up to date care plans per state regulations
- Monitors andmaintainsresident charts and clinical progress notes.
- Communicates to physicians, staff and family changes in resident’s needs or conditions.
- Facilitates coordination of resident services with other service providers.
- Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
- Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
- Communicates in resident’s log any information that staff need to be aware of.
- Provides alternating supervisory visits in coordination with theRN.
- Works as a team player with all members of the residents’ care team.
- Supervises medication practices and medical equipment being used in the building.
- Assures that staff clearlyunderstandand follow all resident care procedures.
- Shares resident and family concerns with pertinent staff ifappropriate.
- Works with manager to assure emergency procedures are in place
LPNQualifications:
- Maintains a current state license as an LPN(Licensed Practical Nurse)per state regulations
- A minimum of oneyear'sexperience in home health, assisted living orlong-termcare is preferred
- Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
- Exhibitsa caring heart with a passion to care for the elderly
