Job summary

Great Lakes Management is seeking an LPN to support seniors in our Minnetonka assisted living community. The role runs 3:00 PM–11:30 PM with a pay range of $31.00–$35.00 per hour, based on experience and qualifications.

LPN certification in MN is required, with a commitment to high-quality resident care and teamwork at the Minnetonka location. Responsibilities include maintaining care plans, monitoring resident charts, coordinating with physicians and families, and ensuring medication procedures

Qualifications

• Current MN LPN license in good standing.
• Minimum 1 year experience in home health, assisted living, or long-term care.
• Strong organizational skills and ability to self-start with minimal supervision.
• Compassion for the elderly and patient focus.

Responsibilities

• Ensure residents meet admission criteria and coordinate placement as needed.
• Maintain up-to-date care plans per state regulations.
• Monitor charts and track clinical progress notes.
• Communicate changes in residents' needs to physicians, staff, and family.
• Coordinate resident services with other providers.
• Oversee medications, renewals, and physician follow-ups.
• Ensure medication administration procedures are followed correctly.
• Document information in the resident log for staff awareness.
• Provide alternating supervisory visits with the RN.
• Collaborate with the care team and supervise medication practices.
• Ensure staff follow all resident care procedures.

LPN Responsibilities:

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents' care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place

LPN Qualifications:

• Maintains a current state license as a n LPN (Licensed Practical Nurse) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
