Job summary

Qualifications

Job Summary

Catholic Charities is seeking a Licensed Practical Nurse (LPN) Health Services Coordinator to provide healthcare oversight and coordination for individuals receiving services in our 245D residential programs throughout St. Cloud and Cold Spring. Our residential homes support adults with developmental disabilities, related conditions, brain injuries, and serious and persistent mental illness. The Health Services Coordinator plays a key role in ensuring individuals receive quality healthcare services, appropriate medication management, and ongoing support to maintain their health and well-being. This position works closely with individuals served, families, guardians, physicians, psychiatrists, pharmacists, and residential staff to coordinate healthcare services, ensure regulatory compliance, and support positive health outcomes.


What you will do

Coordinate and Monitor Healthcare Services


  • Monitor the health needs of individuals receiving services in eight residential homes.

  • Track medical, dental, psychiatric, specialty care, and preventative healthcare appointments.

  • Ensure recommended follow-up care is completed.

  • Monitor changes in health status and communicate concerns to providers and program staff.

  • Assist with the development and implementation of health-related support plans.

  • Track annual physicals, immunizations, dental visits, laboratory testing, and other healthcare requirements.


Manage Medication and Health Supports


  • Provide oversight of medication administration practices across residential programs.

  • Monitor medication effectiveness, side effects, adverse reactions, and potential drug interactions.

  • Review physician orders and ensure medications are administered and documented appropriately.

  • Support compliance with medication administration policies, nursing standards, and Rule 245D requirements.

  • Assist program staff in addressing medication-related concerns and healthcare needs.


Support Behavioral Health Services


  • Coordinate psychiatric services and psychotropic medication monitoring.

  • Track behavioral health follow-up recommendations and required assessments.

  • Monitor psychotropic medication side effects and effectiveness.

  • Communicate concerns to psychiatrists, healthcare providers, and treatment teams.

  • Assist staff in responding to behavioral health concerns and related healthcare needs.


Train and Support Employees


  • Provide orientation and ongoing education related to medication administration and healthcare procedures.

  • Complete medication administration competency evaluations and re-certifications.

  • Conduct CPR and First Aid training as assigned.

  • Serve as a healthcare resource for managers and direct support staff.

  • Identify training needs and support staff development related to client healthcare services.


Ensure Compliance and Quality Assurance


  • Maintain accurate and timely healthcare documentation.

  • Ensure required assessments, consents, and healthcare records remain current.

  • Support compliance with Rule 245D, HIPAA, Minnesota Data Privacy requirements, and agency policies.

  • Participate in audits, licensing reviews, quality assurance activities, and healthcare compliance initiatives.

  • Help maintain systems that promote quality care and positive health outcomes.


What we’re looking for (Required Qualifications)

Education:


  • Current Minnesota Licensed Practical Nurse (LPN) license in good standing.

  • Current CPR and First Aid certification.


Experience:


  • Minimum of four years of related experience.

  • At least one year of experience supporting individuals with developmental disabilities, mental illness, brain injuries, or other vulnerable populations preferred.

  • Experience in residential services, healthcare coordination, medication administration, or long-term care is highly desirable.
