Job summary

Great Lakes Management is seeking an LPN to support seniors in our Roseville, MN assisted living community. The role focuses on maintaining care plans, monitoring residents, and coordinating with the care team.

This part-time position offers 20 hours per week with a competitive hourly wage. The LPN will ensure proper medication administration, communicate changes to physicians and families, and help provide a safe, nurturing environment for residents.

Qualifications

• LPN license valid in MN.
• Minimum 1 year of experience in home health, assisted living or long-term care.
• Good organizational skills with the ability to self-start and follow through with minimal supervision.
• Caring attitude toward the elderly.

Responsibilities

• Ensure residents meet admission criteria and adjust placements as needed.
• Maintain up-to-date care plans per state regulations.
• Monitor charts and document clinical progress.
• Communicate care changes to physicians, staff, and families.
• Coordinate resident services with other providers.
• Oversee medications and coordinate with physicians on changes.
• Ensure proper medication administration procedures are followed and discuss any corrections with staff.
• Document information in residents' logs for staff awareness.
• Provide alternating supervisory visits with the RN.
• Collaborate with the care team to maintain safety and quality.

LPN Job Details:

LPN Responsibilities:

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents' care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place

LPN Qualifications:

• Maintains a current state license as a LPN (Licensed Practical Nurse) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
