Job summary

Humana is seeking a Patient Services Coordinator-LPN to schedule visits, coordinate with field staff and physicians, and support daily office operations. You will ensure care coordination and continuity, assist with staff management, and participate in scheduling and reporting tasks across the branch.

The role requires an active nursing license (RN/LPN) and at least 1 year of home health experience, with strong communication and organizational skills to manage multiple assignments effectively.

Qualifications

• Licensed as an RN or LVN in the practicing state.
• At least 1 year of home health experience preferred.
• Strong communication with patients, families, and staff.
• Excellent organizational and multitasking abilities.

Responsibilities

• Schedule visits for patients and coordinate with field staff and physicians.
• Manage and edit schedules, reassign patients as needed.
• Process infection control forms and report to HRD/Office leadership.
• Maintain hospitalization logs and share updates with office, field, and sales staff.
• Assist with internal transfers and handling lab reports and medical records.
