Job summary

Valor Healthcare is seeking a Licensed Practical Nurse to join the Community Based Outpatient Clinic (CBOC) in Cranberry Township. You will provide prescribed medical treatment and personal care as directed by the physician or mid-level provider, collaborating with the PACT team and extended VA services involved in developing the patient care plan.

Responsibilities include patient care, administering medications, operating diagnostic equipment, maintaining EMR records, and delivering education

Qualifications

• Diploma or certificate of an accredited practical or vocational nursing program.
• Minimum one-year experience as an LPN/LVN in a primary care or ambulatory setting; government healthcare environment preferred.
• Must be credentialed and in good standing with VA.
• Must hold a valid, unrestricted nursing license in the state of employment or applicable VA contract.
• Current Basic Life Support (BLS) certification and ACLS as required by VA contract.
• Proficiency in written and spoken English.
• Strong computer skills, including EMR experience.
• Energetic and optimistic demeanor.
• This is a safety sensitive position.

Responsibilities

• Assist with patient care, examinations, and routine tests.
• Administer medications as ordered and document in patient charts.
• Operate EKG and other diagnostic equipment; perform related tests.
• Input patient information into VISTA/CPRS and maintain clinical reminders.
• Provide patient and family health education and social work referrals as needed.
• Maintain confidentiality of veterans' data and comply with training requirements.
• Support front desk duties as needed and assist in co-managed care with VA providers.
