Qualifications

• Diploma or certificate of an accredited LPN program.
• Minimum one year LPN/LVN experience in primary or ambulatory care.
• Current valid state LPN license; BLS; ACLS as required.

Responsibilities

• Provide patient care and assist with physical exams and tests.
• Perform nursing assessments and preventive health services.
• Administer medications and document in EMR.
• Operate EKG and other diagnostic equipment; collect specimens.
• Maintain confidentiality and patient education on self-care.
• Support patient enrollment and clinic outreach activities.
